Athletic Policies and Registration

Requirements for athletic participation:

  • A valid physical - Submitted once per year and valid for 13 months.

  • A baseline Impact Test - Taken Freshman & Junior year only.

  • Registration through Family ID - Includes permission, medical history and concussion information.


All students interested in playing a fall sport must register through the online FAMILY ID Program and have and up-to-date IMPACT test.  This Family ID registration includes:

  • Registration and health information (required annually)

  • Scanning and uploading a valid student physical (required annually)

  • Submission of required concussion information (required each athletic season)

  • Selection of the sport of the student’s choice (required each athletic season)

Students who registered with FAMILY ID for a sport in 2017-18, still need to register for the fall sport of their choice, and submit the required concussion information!


IMPACT Testing

Any student in Grades 9 or 11, or any transfer students, who did not register for IMPACT Testing (concussion baseline testing) must do so through Athletic Trainer, Mike Cordeiro, at  Mr. Cordeiro will be conducting the IMPACT test by appointment on the following dates and times:

  • Wed. 8/15 at 2pm and 3pm
  • Thurs. 8/16 at 2pm and 3pm
  • Fri. 8/17 at 2pm and 3pm
  • Mon. 8/20 at 2pm and 3pm
  • Tues. 8/21 at 2pm and 3pm
  • Wed. 8/22 at 2pm and 3pm

Families should make an appointment for the test of their choice, and arrive 15 minutes prior to testing. All tests will begin on time and will be conducted in the Art Room, located near the business office.
